Most root canals succeed and last a lifetime — but occasionally, a treated tooth doesn’t heal as expected or develops new problems months or even years later. That doesn’t mean the tooth is lost. Endodontic retreatment gives your natural tooth a second chance, and it’s among the most technically demanding procedures in endodontics — exactly the kind of case where the advanced training of a Board Certified Endodontist can make a difference.

A previously treated tooth can develop problems for several reasons: unusually narrow, curved, or extra canals that went undetected in the original treatment; a delay in placing the final crown that allowed new contamination; new decay or a cracked filling exposing the canal system to bacteria; or a fracture in the tooth itself. Persistent discomfort, new swelling, or a lesion visible on an X-ray around a treated tooth are the common signals.

Dr. Roque begins with 3D CBCT imaging to see what standard X-rays miss — hidden canals, the quality of the previous fill, and the anatomy around the root. During treatment, he reopens the tooth, removes the prior filling material, locates and disinfects any untreated anatomy under the surgical operating microscope, applies Odne Clean and EdgePRO laser-assisted disinfection, and reseals the canal system with bioceramic materials. When retreatment through the crown isn’t the right approach, apical microsurgery may be recommended instead — we’ll walk you through exactly why one path fits your tooth better than the other.
