Centre Endodontics – Terms of Service Effective Date: July 17, 2026

Welcome to Centre Endodontics, a specialty endodontic practice in Wellesley, MA, led by Board Certified Endodontist Dr. Miguel A. Roque, DMD, MMSc. Please carefully review the following terms and conditions governing your use of our services, website, and interactions with our practice. By accessing our services or scheduling an appointment, you agree to abide by these Terms.

  1. General Information

Centre Endodontics is a specialty endodontic practice located in Wellesley, MA, dedicated exclusively to saving natural teeth. Our services include root canal treatment, endodontic retreatment, apical microsurgery (apicoectomy), cracked tooth diagnosis and treatment, care of traumatic dental injuries, and the diagnosis of tooth and orofacial pain, supported by advanced technology including 3D CBCT imaging, surgical operating microscopes, and laser-assisted disinfection. These Terms apply to all patients, visitors, and users of our services.

  1. Appointments
  • Scheduling: Appointments can be scheduled via phone at (617) 410-8595, our website, or in person. Many of our patients are referred by their general dentist; a referral is welcome but not required.
  • Cancellations: We kindly request a 24-hour notice for any cancellations or rescheduling of appointments to avoid a cancellation fee.
  • Late Arrivals: If you arrive late for your appointment, we may need to reschedule or shorten the appointment to accommodate other patients.
  1. Payment and Insurance
  • Payment: Centre Endodontics is a fee-for-service practice. Payment for services is due at the time of treatment unless prior arrangements are made. We accept cash, checks, and major credit cards (Visa, MasterCard, American Express).
  • Insurance: We are an out-of-network provider for dental insurance plans. Most plans offer out-of-network benefits for endodontic care, and as a courtesy we will submit a claim to your insurance carrier on your behalf so that you receive the maximum benefit available under your plan. You are responsible for confirming your coverage and understanding your benefits; any amounts not reimbursed by your insurance remain your responsibility.
  • Estimates and Financial Questions: We are glad to provide a pre-treatment estimate. If you have questions about fees or payment, please contact our office before your appointment.
  1. Patient Responsibilities
  • Accurate Information: You agree to provide accurate, current, and complete information regarding your medical history, dental condition, and insurance coverage.
  • Follow-Up Care: It is your responsibility to follow the care instructions provided by our clinical team, attend any necessary follow-up appointments, and return to your general dentist promptly for any recommended final restoration (such as a crown) following endodontic treatment.
  • Conduct: You agree to behave respectfully while in our office and to cooperate with our staff during your visits.
  1. Privacy and Confidentiality

We are committed to protecting your privacy. All patient information is stored securely and used solely for purposes related to your care and treatment. We comply with all applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

  1. Treatment Consent
  • Informed Consent: By scheduling an appointment, you give consent for our clinical team to examine you and assess your oral health and to recommend appropriate endodontic treatment.
  • Risks and Benefits: Before any procedure, our team will inform you of the risks, benefits, and alternatives to treatment. You have the right to ask questions and make an informed decision regarding your care.
  1. Promotional Information Consent

By using Centre Endodontics services, including signing up for text message communications, you expressly consent to collecting, using, and sharing your personal information as outlined in our Privacy Policy.

You further consent to receive text messages from Centre Endodontics including transactional and promotional messages.

7a. Message Frequency and Content

By subscribing to Centre Endodontics text messaging service, you agree to receive promotional, transactional, or informational messages related to our services and updates, such as appointment reminders, post-treatment instructions, and requests for feedback. You can expect to receive up to 10 messages per week. Message and data rates may apply.

7b. Opt-out Procedures

You may choose to stop receiving promotional text messages from Centre Endodontics at any time. To opt out, simply reply to any text message you receive from us with the word ‘STOP’ or ‘UNSUBSCRIBE.’ Once we receive your opt-out request, we will promptly remove your number from our promotional messaging list. Please note that opting out of promotional messages will not affect your ability to receive important service-related communications.

7c. Liability and Disclaimers

Centre Endodontics is not responsible for any delays, failures in delivery, or any other issues related to the transmission or receipt of text messages. Delivery of text messages is subject to effective transmission by your mobile carrier and is not guaranteed by Centre Endodontics.

By subscribing to our SMS services, you acknowledge and agree that Centre Endodontics will not be liable for any damages, losses, or injuries arising from or related to the use or failure to receive any text messages, including but not limited to, delays, non-delivery, or technical issues. Your use of our SMS services is at your own risk, and we provide our services on an ‘as-is’ basis without any warranties of any kind, express or implied.

Centre Endodontics is committed to protecting the security of your personal information. However, it is also your responsibility to protect the confidentiality of your account information, and any passwords associated with your use of our services.

You agree to notify Centre Endodontics immediately of any unauthorized use of your account or any other security breach. Centre Endodontics will not be liable for any loss or damage arising from your failure to protect your account or personal information adequately. By using our services, you acknowledge and accept that no data transmission over the Internet or mobile networks can be guaranteed to be 100% secure, and therefore, you use our services at your own risk.

  1. Coordination of Care and Referrals

As a specialty practice, we work in close partnership with your general dentist and other dental professionals. Where care outside the scope of endodontics is needed, we may refer you back to your general dentist or to another appropriate specialist. We are not responsible for the services provided by other providers, but any referral we make will be made with your best interests in mind.
